Program Waitlist Hub

 

The TAF Program Waitlist Hub allows patients to see the status of their waitlist application and accept or decline their enrollment invitation. To use this tool, enter the patient’s last name, date of birth, and waitlist PIN, which the patient should have received after completing the waitlist application. If you do not know the patient’s PIN, please contact a TAF Patient Advocate at (855) 845-3663.
